The Dallas location, Forest Park Family Medicine, has permanently closed, effective 1 March 2017. All future appointments will be booked in the Allen office.

Please call 911 or proceed to the nearest emergency department if you or a family member are experiencing one of the following emergencies:
Severe pain, uncontrolled bleeding, a rectal temperature over 101° in an infant under five months, an oral temperature above 104° in an older child or an adult, vomiting blood or passing blood from the rectum, poisoning, accidents resulting in loss of consciousness, first episode of seizures or convulsions, chest pain suggesting heart attack (mid-chest pressure with sweating, nausea or arm pain), increasing angina in heart patients, and strokes (focal weakness, numbness, slurred speech, seizures, and/or loss of consciousness).
Most other problems (sore throat, rash, diarrhea, mild headaches, conjunctivitis) can safely wait until the next morning, but please call if you have doubts. If you call the office for an emergency, please have a pharmacy number available for the medical assistant or physician. When calling after hours, please have the following information available: your name, the patient's name, a phone number where you can be reached, the nature of your concern and a pharmacy phone number. This will enable the on-call physician to reach you and to call in a prescription if necessary.
